Referring a patient early allows us to provide timely care and greater support for both patients and their families.
Referrals can be made by physicians, family members, loved ones, or even by patients themselves. However, a physician’s medical authorization is required before care can begin.
We accept patient admissions 24/7, and all personal and medical information is handled with strict confidentiality under HIPAA regulations.
Our admissions team will contact the patient and/or their family within 24 hours to assess needs and determine the most appropriate level of care.
If the patient qualifies, we will help coordinate physician orders and complete all required documentation to begin services smoothly. Once care starts, our skilled clinical team collaborates closely with the patient’s physician to deliver coordinated support and achieve the best possible outcomes.
